Mental Health Awareness Month


During Mental Health Awareness Month it is fitting to not only think about those who struggle with mental health issues, but also to take a look at how Tabor is working with individuals with a mental health diagnosis.

The PATH program at Tabor is a supportive housing program that assists individuals with a serious mental illness to find independent housing and provides intensive support to those individuals for nine months. PATH stands for “Projects for Assistance in Transition to Homelessness." It is a program of SAMHSA (Substance Abuse and Mental Health Services Administration), which is an agency within the U.S. Department of Health and Human Services.

The PATH program at Tabor uses the Critical Time Intervention Model, which was created to prevent homelessness in individuals with mental illness following a discharge from hospital, shelters, and other institutions. It is set up to assist these individuals in their transition to community.

The clients in this program are either homeless or at risk for homelessness, and need intensive support to transition to community living. They need assistance to find housing and then to connect to resources and learn how to maintain housing. Each individual comes with varying levels of independence. Some need more intensive services throughout the program, and others may be able to make it alone fairly quickly. The key is getting to know the individual, meeting them where they are, and walking with them during this time of transition.

The most important thing to keep in mind, when given the opportunity to assist anyone who may be struggling with mental health issues or any other challenge, is to have empathy and treat them with love and respect.
