Rapid Re-Housing Case Manager conducts a comprehensive housing assessment and sends referral to CHS




CHS meets with clients to begin housing search




CHS and Client look for housing, Housing Location Specialists advocate for clients and assist with lease signing




Client resumes working their Housing Case Manager for 6-12 months of supportive services
